Label type
paper is the most common, the printed paper adhered to the melted shellac in the mold.
paste-over means pasted on after the record was pressed
pigmented means ink was pressed into the record surface
embossed means the mold was used to press the information into the surface
etched is for metal disks where the label image was created by acid through a mask
foil is like paper above, but a metal foil is used instead of paper.
Groove type
Is the music encoded by varying the groove horizontally (lateral), or vertically?   Vertical is found on most thick Edison disks.  Or is the groove universal, meaning both vertical and horizontal movement is found.
Country Country where disk is manufactured.  If made for sale in another country, enter the sale-country after the manufactured-country separated a slash.  For example,     England/India.      If you are not sure, leave blank.  If only country of manufacture is obvious, put just that country name, e.g if it says 'Made in Belgium'  just put the word Belgium
